Ada Exempted Schools has partnered with FinalForms, an online forms and data management service which allows you to complete and sign emergency, academic and athletic forms for your students. FinalForms saves data from season-to-season and year-to-year, meaning that you will never need to enter the same information twice! FinalForms also pre-populates information wherever possible, for each of your students, saving you time.

You will review your information once per school year to verify information is current and sign the forms.

The Parent Playbook (below) is a great way to get started in FinalForms. If you require any support during the process, scroll to the page bottom and click “Use Support”.

All forms should be completed and signed before the first day of school. Please register here if you do not yet have an account!

For high school (grades 7-12) students are required to have their Emergency Medical Form completed prior to schedule pickup. Students who do not have their forms completed at their schedule pickup time will not receive a schedule.
