FinalForms
We are very excited to announce that the Ada Exempted Schools has partnered with FinalForms, an online forms and data management service. FinalForms allows you to complete and sign emergency, academic and athletic forms for your students. The most exciting news is that FinalForms saves data from season-to-season and year-to-year, meaning that you will never need to enter the same information twice! FinalForms also pre-populates information wherever possible, for each of your students, saving you time.
You will review your information once per school year to verify information is current and sign the forms.

We are asking that ALL parents of students use FinalForms beginning for the 2018-19 school year. All forms should be completed and signed before the first day of school. You may create your account and sign your forms immediately. Please register here now and electronically complete ALL forms! You will also find a link to FinalForms on our website as well.
For high school (grades 7-12) students are required to have their Emergency Medical Form completed prior to schedule pickup. Students who do not have their forms completed at their schedule pickup time will not receive a schedule.
